Jesse
Thomas is the President of Molina Healthcare of Michigan, Inc., and formerly
was the President of Molina Healthcare of Ohio, Inc. Jesse has over 25 years of
experience in the healthcare industry and has held various leadership roles in
such key functions as business expansion, organizational turnarounds, strategic
planning, problem solving, public policy and community advocacy. Prior to
joining Molina Healthcare of Ohio, he was appointed the Chief/Administrator of
the Office of Healthcare Purchasing, Illinois Department of Healthcare and
Family Services, where he was responsible for leading the integration of the
healthcare procurement function of four State Agencies.
Also, prior to joining Molina, Jesse was the Executive Director of United
Healthcare Illinois - AmeriChoice; President and CEO of HealthPlans of America,
NJ, Inc; Vice President/Director Major Account Sales at Blue Cross and Blue
Shield, NCA; and President of Total Health Care, Inc. to name a few.
Additionally, Jesse has served as the Board President for the Illinois
Association of Medicaid Health Plans in Chicago. He received his Bachelors
degree in Liberal Arts & Sciences from the University of Illinois; and,
Graduate Studies in Management and Accounting from Eastern Michigan University.
Jesse is a former candidate for the U.S. Congress from Colorado where he ran on
a platform of improving quality and access to affordable healthcare for all. He
was subsequently appointed to several State Boards. Also, he was the Governor's
nominee for a Presidential appointment to the U. S. Department of Health and
Human Services.

| Nancy Wanchik, MSHA |
Nancy
Wanchik is the Chief Operating Officer for Molina Healthcare of Michigan and is
responsible for Member Services, Enrollment Services, Administrative Services,
Community Affairs, Provider Services, Network Development, and Government
Contracts. Prior to joining Molina, she was the President and Chief Operating
Officer for CAPE Health Plan, an HMO serving Medicaid beneficiaries in
Southeast Michigan. CAPE Health Plan was acquired by Molina in 2006. Ms.
Wanchik has over 25 years of leadership experience in health care spanning
professional practice management, institutional department management, and
managed care operations. She has served in several leadership roles for the
Michigan Association of Health Plans. Ms. Wanchik is active in the community
and currently serves on the board of St. Frances Cabrini Clinic, the oldest
free clinic in the United States.
Ms. Wanchik holds a Masters of Science degree in Health Services Administration
from Central Michigan University, and a Bachelors of Science in Health Services
Administration from Ferris State University.

| James D. Forshee, M.D. |
James
D. Forshee MD is the Chief Medical Officer (CMO) responsible for medical
management with oversight responsibilities for utilization management, quality
improvement, credentialing, pharmacy, and risk management activities. Dr.
Forshee has served as the Regional Medical Director in Michigan for
Correctional Medical Services, Vice President of Medical Affairs for Centene
Corporation in Ohio, Medical Director for United Healthcare at Community Choice
Michigan, Medical Director for Blue Care Network, and Senior Vice-President and
CMO for SelectCare. Dr. Forshee is a member of the American Medical
Association, the American College of Physician Executives, Michigan State
Medical, the Society of Chief Medical Officers, the American Academy of Family
Physicians, and the Michigan State Medical Society Advisory Committee of
Medical Economics. He received his MBA degree from the University of Michigan
and his MD degree from Michigan State University. Dr. Forshee is Board
Certified in Family Practice.

Christine Surdock, Chief Financial
Officer for Molina Healthcare of Michigan (MHM), has 20 years of financial
management experience in healthcare. Ms. Surdock was Molina Healthcare's former
director of Finance for two years. She is responsible for directing all
accounting and IS activities for the company. Prior to her role at Molina
Healthcare, Ms. Surdock served in various financial and administrative
positions in Michigan for The Wellness Plan, Care Choices, and SelectCare, Inc.
Ms. Surdock received her Bachelors of Business Administration and MBA from
Eastern Michigan University, Ypsilanti, Michigan.
