Drug Formulary

Getting My Medicine

Molina Healthcare uses a list of approved drugs. This is called a "drug formulary." This list includes both prescription and over-the-counter medicines. The list is put together by a group of providers and pharmacists. The drugs on this list are quite common. Your provider will most likely prescribe drugs for you from the list.

Certain drugs on the list will need approval from Molina Healthcare. Your provider will need to send Molina Healthcare a request. Then, these drugs can be prescribed to you. Your provider can ask for you to have a drug that is not on the list. He/She will need to send us a request for approval.

If the request is denied, you or your provider can appeal our decision.

Certain drugs, even though they are on the list, may have special limits on the amount that can be filled at your pharmacy. Some may want you to have tried and failed other medicines first. Some drugs are never covered. These may include drugs for weight loss, unless they are found to be medically needed for your condition.

Pharmacies

Molina Healthcare has contracts with certain pharmacies. You must get your drugs at one of these pharmacies. Click here to Find A Pharmacy. You can also call Member Services at 1-866-472-4585. They can help you find a pharmacy closet to you.
