Molina Healthcare of Missouri offers Disease Management Programs that can help you manage your chronic health conditions. Our programs offer learning materials, advice and care tips.

There are many ways you can enroll in a Disease Management Program. One way to enroll is when you receive information from Molina based on the healthcare services that you have obtained. As part of this program you may also have a nurse care manager call you. The nurse will work with you and your healthcare provider to help you stay well.

You may be referred to these programs by:

• Provider referral
• Self-referral

It is your choice to be in these programs. If you do not want you or your child to take part in these programs or you would like more information about our Disease Management Programs, please call Member Services at 1-800-875-0679, Monday through Friday, 8:00am - 5:00pm.
