Approved Drugs

Molina Healthcare uses a list of approved drugs. This is called a "drug formulary." The list is put together by a group of providers and pharmacists. Your provider will generally prescribe drugs for you from the list.

Brand Name Drug Formulary (alphabetical listing)

Certain drugs on the list will need approval from Molina Healthcare. Your provider will need to send Molina Healthcare a request before these drugs can be prescribed to you. If your provider wants you to have a drug that is not on the list, your provider can send us a request for approval.

If the request is denied, you or your provider can appeal our decision.

Pharmacies

Molina Healthcare has contracts with certain pharmacies. You must get your drugs at one of these pharmacies. A list of these pharmacies is in Molina Healthcare's Provider Directory. You can also call Member Services in Albuquerque call 1-505-342-4660 or toll-free 1-800-377-9594 to find out which pharmacy is closest to you.

Over-the-Counter Drug Formulary

30-day supplies of the products on this list are considered part of the Molina Healthcare drug benefit when the same requirements for prescription drugs (e.g., written on a prescription blank) are met.
