Suggested Timeline for Children
Birth to 19 years
These guidelines are suggestions for preventive care. You and your provider may decide to do some services more or less often. Molina Healthcare covers preventive care services.

| Birth to 2 weeks old |
- Possible blood test
- Immunization Hepatitis B

- Immunizations: DTaP, Hepatitis B

| 4 months old |

| 6 months old |
- Immunizations: DTaP, Influenza (yearly beginning at 6 months old if considered high risk and household members)
|
| 9 months old |
|
| 12 to 15 months old |
- Immunizations: MMR, Hib, Varicella (Chicken Pox), Polio, Hepatitis B (between 12 and 18 months)
- Lead screen (at 12 months)

| 15 to 18 months old |
- Immunizations: DTaP, Prevnar, third Polio vaccine by 18 months
|
| 24 months old |
- Immunizations: PPV (for certain high risk groups including NativeAmerican/Alaska Native populations), Prevnar
- Lead screen (at 24 months)

| 3 to 11 Years |
| 3 years old |
|
| 4 to 6 years old |
- Blood pressure check yearly
- Urine Test and vision test once during 4-6 year visits
- Immunizations: DTaP, Polio, MMR (once during 4-6 year visits), and Hep B if not immunized, Hep A (for certain high risk groups), PPV (for certain high risk groups)

| 7 to 11 years old |
- Blood pressure check yearly

| 12 to 19 years |
| 12 to 19 years old |
- Immunization: MMR (if not given at 4-6 year visits), Td (Tetanus and Diphtheria booster), Hep B series (if not received), Hep B series (if not received)
- Blood pressure check every other year
- If sexually active, may be checked for Sexually Transmitted Diseases (STDs)
