Suggested Timeline for Children

Birth to 19 years

These guidelines are suggestions for preventive care. You and your provider may decide to do some services more or less often. Molina Healthcare covers preventive care services.

Your child should see his/her PCP for a Healthy Kids Exam at Age: What to expect in addition to the exam:
Birth to 36 months
Birth to 2 weeks old
  • Possible blood test
  • Immunization Hepatitis B
2 months old
  • Immunizations: DTaP, Hepatitis B, Polio (IPV), HiB, PCV13 and Rotavirus (RV)
4 months old
  • Immunizations: DTaP, Polio (IPV), HiB, PCV13, and Rotavirus (RV)
6 months old
  • Immunizations: DTaP, HiB, PCV13 and Rotavirus (RV), Influenza (yearly beginning at 6 months old if considered high risk and household members)
9 months old
  • Blood test
12 to 15 months old
  • Immunizations: MMR, Hib, Varicella (Chicken Pox), Polio, Hepatitis B (between 12 and 18 months), Hepatitis A and PCV13
  • Lead screen (at 12 months)
15 to 18 months old
  • Immunizations: DTaP, Prevnar, third Polio vaccine by 18 months
24 months old
  • Immunizations: PPV (for certain high risk groups including NativeAmerican/Alaska Native populations), Prevnar
  • Lead screen (at 24 months)
3 to 11 Years
3 years old
  • Blood pressure check
4 to 6 years old
  • Blood pressure check yearly
  • Urine Test and vision test once during 4-6 year visits
  • Immunizations: DTaP, Polio, MMR (once during 4-6 year visits), and Hep B if not immunized, Hep A (for certain high risk groups), PPV (for certain high risk groups), Varicella (chicken pox)
7 to 11 years old
  • Blood pressure check yearly
12 to 19 years
12 to 19 years old
  • Immunization: MMR (if not given at 4-6 year visits), Td (Tetanus and Diphtheria booster) or Tdap booster, Hep B series (if not received), Hep A series (for high risk groups), Meningococcal (MCV) and Human Papillomavirus (HPV – 3 dose series)
  • Blood pressure check every other year
  • If sexually active, may be checked for Sexually Transmitted Diseases (STDs)
