- Answer questions you may have about your illness
- Answer questions about your health plan coverage
- Set goals with you to help you recover from an illness
- Talk to the medical director of the health plan about your care, if needed
- Help you find other sources for things that may not be covered by your health plan
- Help coordinate appointments and tests
- Help coordinate transportation
How can I help my Case Manager help me?
- Write down your questions. This will help you remember things when you have a lot to talk about.
- Tell your Case manager about all of your appointments and tests. Not being aware of all the details of your case can keep the Case Manager from helping you.
When do I call my Case Manager?
Your Case Manager will give you a phone number to call once you have been contacted. If you want a Case Manager, please call Member Services at 1-800-580-2811 or 1-505-342-4681 for assistance.
If you are already open to Case Management and have forgotten the direct phone number of your Case Manager, you may call 1-800-377-9594, extension 181120.
If you feel that you need emergency treatment, please get help first or call 911.
