Molina Healthcare offers Disease Management Programs that can help you manage your chronic health conditions.  Our programs offer learning materials, advice and care tips. As part of this program, you may also have a Molina Healthcare Case Manager call you. The nurse will work with you and your provider to help you stay well.

Members are enrolled based on their medical claims.
