If you must travel 30 miles or more from your home to receive covered health care services, Molina will provide transportation to and from the provider’s office if needed. This transportation benefit is for medically necessary Medicaid-covered services that are not available from a provider closer to your home.

When you do not have other transportation available, Molina can provide transportation to health care appointments that are less than 30 miles from your home as an additional benefit. Molina provides 15 round-trip visits (30 one-way trips) for each member per 12-month rolling period to any Molina provider, WIC or CDJFS re-determination appointment.

In addition to the transportation assistance that Molina provides, members can still receive assistance with transportation for certain services through the local County Department of Job and Family Services Non-Emergency Transportation (NET) program. Call your County Department of Job and Family Services for questions or assistance with NET services.

To arrange transportation, or if you have any questions, please call 1-866-642-9279 (TTY for the hearing impaired: 1-800-750-0750 or 711) for assistance. Please call as soon as possible to schedule your transportation, but no later than 48 hours in advance of your appointment.

Plan ahead!

Molina may not be able to schedule your transportation if you do not call at least 48 hours in advance.
