Molina Healthcare has programs that reach out to members at risk for certain health conditions.
Our programs offer learning materials, advice, care tips and provider help. The goal is to help you or your child learn to manage your health.
If you have any of the health conditions listed below you will be enrolled and will begin to receive learning materials and newsletters. If you or your child's condition is more severe, you may receive a call from a Care Manager. This care manager will work with your provider to make sure you have what you need to stay well. (Members are found through claims, pharmacy data or by your or your child's provider.)
