Welcome and thank you for choosing Molina Healthcare of Utah as your Health plan.

We know your health is important to you. We will do all we can to help you and your family with your health care needs.

This is an important booklet. It will tell you how to get healthcare through your Molina Healthcare plan. Read this booklet carefully. Please look closely about how to choose a Primary Care Physician (PCP) and what to do if you have a medical emergency. Your PCP will be in charge of your medical care.

It is very important that we know how to contact you. If you move or get a new telephone number, please call Member Services at 1-888-483-0760. You can call Monday through Friday from 9:00 a.m. to 5:00 p.m. Always, tell your Medicaid Eligibility Worker (Caseworker) about these changes as soon as you can.

You will get a copy of this booklet at least once a year. You can also call Member Services if you have a problem or a complaint.
