Welcome and thank you for choosing Molina Healthcare of Utah as your health care provider.
We know your health is important to you, and we will do all we can to help you and your family with your health care needs.
CHIP stands for Children's Health Insurance Program. CHIP offers medical insurance to children under 19 years of age, who don't have access to health insurance or Medicaid.
This booklet contains information on the services that are available to you as a CHIP member of Molina Healthcare. Please review this booklet carefully. In your review, please give extra attention to those sections that tell you how to choose a doctor as your Primary Care Physician (PCP) and what to do if you have a medical emergency. Your PCP will be in charge of your medical care.
It is very important that we know how to contact you. If you have a change of address or telephone number, please call Member Services between the hours of 9:00 a.m. and 5:00 p.m., Monday through Friday, at 1-888-483-0760. Also, please notify your CHIP Eligibility Worker of these changes as soon as you can.
Please call Member Services for any questions or information you may need, or to inform us of any concerns or complaints you may have with our services. You will get a copy of this booklet at least once a year.

Language Translation Services
(Talking to someone who speaks your language)
We know that it may be hard to understand what your doctor is telling you if your first language is not English or if you are hard of hearing. We may have doctors who speak or sign your language. You may ask to receive services from them.
We also contract with interpreters who will help you to talk to your doctor and understand what your doctor or health plan tells you. Interpreters are available in all languages and are free of charge. Molina Healthcare will get an interpreter to help you over the telephone or to meet you at medical appointments. If you need an interpreter call 1-888-483-0760 and a Member Service Representative will get one for you.
If you are deaf and need the help of a signer call Member Services at 1-888-483-0760 we will get one for you.
If you have a hard time speaking, you can call 1-888-346-5822 and a specially trained person will help you.
If you are hard of hearing, you can call Utah Relay Services at 711 or 1-800-346-4128. This is a telephone relay service or TTY/TDD that is a free public service. Si usted habla español, usted puede llamar - Spanish Relay Utah at 1-888-436-3162.
