If you do not have your own transportation to and from your medical appointments, you can ask for a Utah Transit Authority (UTA) bus pass by calling your Medicaid Eligibility Worker.

If there is a medical reason you can’t use the bus, you may qualify for services through UTA FlexTrans. To apply for this service call the Medicaid Information Line at 1-801-538-6155 or 1-800-662-9651 and ask for the Transportation Unit.
