Molina Healthcare of Washington, Inc. (Molina Healthcare) offers Disease Management Programs that can help you manage your chronic health conditions. Our programs offer learning materials, advice and care tips.

Members are enrolled based on their medical or pharmacy claims. As part of this program you may also have a nurse care manager call you. The nurse will work with you and your provider to help you stay well.
