Health Promotion Programs

Molina Healthcare of Washington is here to help you stay well.

We have many health promotion programs and services to offer you. There are many ways you can be referred to a program or service. One way to enroll is through your provider. You also can self-refer to the program by calling Member Services at (800) 869-7165 (Monday through Friday, 8:00 a.m. - 5:00 p.m.). There are certain requirements you must meet. If you (or your child) are enrolled and do not want to take part in the program, please call us at the number listed above.

QI Incentives

Prenatal Car Seat Program:
Pregnant mothers who qualify receive a gift card for a car seat or booster seat if timely prenatal care is received.
Postpartum Program:
A $5 gift card is offered to new mothers who have a postpartum visit within eight weeks after delivery.
Well-child/Adolescent Birthday Program:
During the month of their birthday, members ages 1-13 receive a birthday letter. This letter reminds them to see their Primary Care Provider (PCP) for a yearly exam. When you take your child to the yearly exam, take the birthday letter with you. Have the PCP sign the letter and fax it back to Molina Healthcare of Washington at (800) 461-3234. Once we receive the completed form your child can receive a gift from us:
Preventive Services Programs:
There are gift card incentives for members who are enrolled in the Healthy Living with Diabetes disease management program. Members who receive a diabetes eye exam also known as dilated retinal exam (DRE) and have their A1c, cholesterol and kidneys tested can receive gift cards worth up to $20!

Women between the ages of 40-69 can receive a $10 gift card for getting a mammogram.
