Disability and Senior Access Services

Health and wellness is the same for people with or without disabilities or a​ctivity limitations. Molina Healthcare's Bridge2Access Program will help improve access to health care services for all members. Here are a few things we offer our members:

Bridge2Access Services

  •  Physical Access
    • Online and written directory with access indicators
    • Member services support to find an accessible provider

     

    To find an accessible Molina Healthcare provider near you, visit Find a Provider, look for the universal access symbol and search based on your access needs or call Member Services.
  •  Communication Access
    • Sign language and foreign language interpreters at no cost.
    • Materials in alternate formats (Large Print, Braille and Audio, and more)

     

    To request an interpreter for your doctor’s appointment, call Member Services 3-5 days before your appointment.

  •  Coordinated Care Team
    • Access to a care manager and team who can help you coordinate and get the most from your health benefits.
    • Help you transition from hospital or other facility to home
    • Coordinate services with other agencies, like IHSS, CBAS, MSSP, Long Term Care and more!

  •  Transportation Services
    • Molina Healthcare shuttles in some communities.
    • Learn what about the transportation services you may be eligible for.

Meeting the healthcare needs of people with disabilities and older adults is very important to Molina. Member and stakeholder input plays a vital role in shaping healthcare programs.

    AAA is a separate division within the Aging and Adult Services Branch. It is responsible for identifying unmet needs as well as planning, coordinating and implementing programs that promote the health, dignity and well-being of our older residents.     The In-Home Supportive Services (IHSS) Program helps pay for services provided to low-income elderly, blind or disabled individuals, including children, so that they can remain safely in their own home. IHSS is considered an alternative to out-of-home care, such as nursing homes or board and care facilities.

    The Home Delivered Meals program is designed specifically for home-bound individuals. This valuable program not only delivers a nutritious meal on a daily basis, but also serves as a mechanism to check up on the client, thereby providing an additional safety inspection and furthering the ability for him or her to remain in the home
