Home and Community-Based Services (HCBS)

Waiver services provided for home and community based health care services for aged and disabled persons. The program's goal is to allow persons to remain at home to receive health services. If they did not receive such services, these persons would require nursing home care. The waiver may provide other benefits to help the person remain at home.

What are home and community-based services?
HCBS provides a variety of services to help you live safely in your own home.

What services are included in HCBS?
Examples of services you may be able to receive include:

  • Medical equipment and supplies
  • Home delivered meals
  • Chore services
  • Environmental modifications
  • Adult day program
  • Personal emergency response system

 

How do I get HCBS?
