  • What is the Hybrid Preferred Drug List?
    • Starting January 1, 2026, Molina and the other Medicaid Health Plans must use a shared or “hybrid” drug list with Utah Medicaid for certain drug classes. Molina must follow all requirements from Utah Medicaid including which drugs are preferred and non-preferred, any limitations including prior authorization, and clinical criteria.
    • The Hybrid Preferred Drug List will include the following drug classes:
      • Antidiabetic drugs including insulin
      • Asthma and Chronic Obstructive Pulmonary Disease (COPD) inhalation Drugs
      • Atopic Dermatitis (non-steroidal) Drugs
      • Continuous Glucose Monitors (CGMs) and Diabetic Supplies
      • Hepatitis C—Direct-Acting Antivirals (DAAs)
      • Human Immunodeficiency Virus (HIV) Drugs
      • Immunomodulators
    • If a drug you are currently taking is impacted by these new requirements, Molina will send you and your provider a letter.
    • In some cases, you may be able to stay on your current medication. This is called “grandfathering”. 
    • If your provider does not want you to change drugs, they can submit a Prior Authorization to Molina and tell us why you need to stay on your current drug.
