Welcome, Illinois Medicare-Medicaid Plan Healthcare Providers

Molina Healthcare is participating in a multi-year demonstration program between the Centers for Medicare & Medicaid Services (CMS) and the Illinois Department of Healthcare and Family Services (HFS). This is known as the Medicare-Medicaid Alignment Initiative (MMAI) in Illinois. This program will benefit our members by providing the convenience of coordinated care with one primary care provider and one ID card.

Molina Healthcare's Medicare-Medicaid Plan (MMP) is called Molina Dual Options.

Contracted providers are an essential part of delivering quality care to our members. We at Molina Healthcare value our provider partnerships and support the doctor-patient relationship our members share with you.

Molina’s Provider Self Services has a variety of tools to simplify your transactions whether you need to check eligibility or check claim status. For more information contact Provider Services. Molina Healthcare is now collecting National Provider IDs (NPIs) from contracted and non-contracted providers. Submit your NPI to Molina via our Provider Self-Services Portal.

Reminder: Molina would like to remind you to visit our News & Updates page in order to access the Provider Bulletin and other important updates on prior authorization and billing requirements. To access this page, visit the Communications tab located on the website’s navigation bar and select News & Updates.

       office hours, phone, fax, or email
  • • Addition or closure of office location
