Molina’s Provider Self Services has a variety of tools to simplify your transactions whether you need to check eligibility or check claim status. For more information contact Provider Services. Molina Healthcare is now collecting National Provider IDs (NPIs) from contracted and non-contracted providers. Submit your NPI to Molina via our Provider Self-Services Portal.

Thus, we are looking for highly qualified practitioners.

How To Join

Please send the completed documents listed below to or by facsimile to (877) 814-0342.
  • Copy of state license

  • Copy of liability insurance

  • Completed ownership form

  • Copy of W9 for the practice

  • Completed Release form

  • You must have your NPI registered with the Health Care Authority (HCS). This is a federal requirement in for our plan to enroll providers. If you have questions regarding this requirement, please contact 1-800-562-3022 ext. 16137.

  • HDO (Healthcare Delivery Organization) application for Facilities

  • Completed Credentialing application (CAQH) Molina prefers that you use CAQH whenever possible Credentialing documents uploaded to CAQH or Provider Source (OneHealthPort) streamlines the process. If you have your information loaded to CAQH or Provider Source (OneHealthPort), we can obtain most documentation needed from either of those sources. Please give us your CAQH# or Provider Source#.

The credentialing process includes: 1) Primary source verification of information in the application; 2) Verification of the Disclosure of or Change in Ownership and Control Interest Form and 3) Our credentialing Committee’s review and final determination. This process may take up to ninety (90) days from the time we receive complete credentialing requirements. When the credentialing process is complete, you will receive written notification of the results from our Credentialing department.

In order to see Molina Members, providers must complete the credentialing process and return a signed contract to our contract department.

If you are approved to be part of the Molina Healthcare provider network, our Provider services department will notify you of your effective date by letter and send you one fully-executed agreement. Please note, you may not begin seeing Molina Healthcare members until you have received this notification. Shortly after approval notification has been sent out you will be contracted by our Provider Services department to schedule an orientation. Reminder: Molina is GOING GREEN as of July 1st and all claims need to be submitted through an EDI Clearinghouse or via Molina's Provider Portal. Please contact our Provider Services team if you have any questions.

Please notify Molina Healthcare at least 30 days in advance when you have any of the following:

       office hours, phone, fax, or email
  • • Addition or closure of office location
  • • ​Addition or termination of a
  • • Change in Tax ID and/or NPI

       patients (PCPs only)
