These guidelines are suggestions for preventive care. You and your provider may decide to do some services more or less often. Molina Healthcare covers preventive care services.

Cholesterol screen
  • Beginning at 35 for men and 45 for women

Colon and rectal cancer screen

  • Sigmoidoscopy or Colonoscopy
  • Stool blood check
  • Every 5 years
  • Yearly
  • Every 5 years
Especially for Women

Breast cancer screen

  • Self breast exam
  • Mammogram and breast exam by provider
  • 20 + years old
  • 40 + years old
  • Monthly
  • Every 1 -2 years or as advised by your provider

Cervical cancer screen

  • PAP smear
  • Chlamydia screen (sexually transmitted disease)
  • 19 + years old
  • Every year until 2 exams in a row normal then every 3 years, as long as your PAP results stay normal
  • Yearly, if sexually active and after age 25, yearly if high risk
Especially for Men
Testicular cancer self examination
  • 20 + years old
  • Monthly
Prostate cancer screen
  • 50 + years old
  • Every 1 - 2 years
Immunizations
Rubella antibody check
  • Prior to first pregnancy
  • Once
Diphtheria-Tetanus booster
  • 19 + years old
  • Every 10 years
Human papillomavirus (HPV)
  • This immunization series is
    covered by DSHS Fee For Service for Women 19 & 20 years
 
Influenza vaccine (Flu shot)
  • 65 + years of Age,
    or if you have a chronic illness, ask your provider
  • Yearly
Ask Your PCP About
Hepatitis A and B
  • If you have a special risk, ask your provider

One series of shots:

  • 2 for Hepatitis A
  • 3 for Hepatitis B
Pneumococcal vaccine
  • 65 + years of Age, or if you have a chronic illness, ask your provider
