11/26/2013

Molina Healthcare announced that the three health centers it manages on behalf of Fairfax County had been recognized by the National Committee for Quality Assurance as part of the Patient-Centered Medical Home 2011 program. All three of the centers were recognized at Level 3-the highest level of PCMH recognition, making them one of the only three safety-net clinics in northern Virginia with this NCQA distinction. This unique public-private partnership with Fairfax County and Molina Healthcare has been a proven model of providing excellent comprehensive primary care to a medically needy and vulnerable population.

“The NCQA Patient-Centered Medical Home is a model of 21st century primary care that combines access, teamwork and technology to deliver quality care and improve health,” said NCQA President Margaret E. O’Kane. “NCQA’s PCMH 2011 Recognition shows that the three health centers run by Molina Healthcare, have tools, systems and resources to provide its patients with the right care at the right time.”

The three health centers of Fairfax County’s Community Health Care Network have fifteen primary care providers, in-house laboratories, extensive onsite pharmacy including onsite medication counseling, integrated behavioral health, and most recently, a new telehealth program. Together, these health centers provide approximately 52,000 comprehensive primary care visits per year.
