Here is what happens next.

  • You will receive a notification from the Human Services Department letting you know if you are enrolled for Centennial Care.
  • If you are enrolled, Molina Healthcare will send you a Member Welcome Packet and Member ID Card 
    • The Member ID Card and welcome packet are sent in two mailings.
    • Call Member Services if you have questions. You can call us Monday to Friday (except on holidays), 8 a.m. - 5 p.m.

You may have to pay for services that are not covered. You may also have to pay for services from providers not part of our network. If the services were an emergency, you don’t have to pay. If you need help, call Member Services.
