Passport Advantage (HMO D-SNP)
Passport Advantage (HMO D-SNP) is a plan for people who qualify for Medicare and Medicaid.
You are eligible to join the Passport Advantage (HMO D-SNP) plan if:
- You reside in the Passport Advantage (HMO D-SNP) geographic service area
- and – you must you must have both Medicare Part A and Medicare Part B
- and – you must be entitled to Medicare cost sharing assistance under Medicaid
- – and – you are entitled to Medicaid

Passport Medicare Choice Care (HMO)
Passport Medicare Choice Care (HMO) is a Medicare Advantage Prescription Drug (MAPD) plan for members age 65 and older. This plan offers additional supplemental benefits more than what are covered by Original Medicare.You are eligible to join the Passport Medicare Choice Care (HMO) plan if:
- You reside in the Passport Medicare Choice Care (HMO) geographic service area
- – and – you are entitled to Medicare Part A
- – and – you are enrolled in Medicare Part B; and continue to pay your Medicare Part B premium
