2017 Provider Satisfaction Survey

Molina Healthcare is conducting our annual provider satisfaction survey from April 28, 2017 to June 9, 2017. Participating providers are randomly selected to participate. The survey will provide an opportunity for providers to share their opinions about the programs and administration of the plan.

The survey will be delivered by mail to randomly selected providers. Recipients are encouraged to provide valuable feedback by taking a few minutes to complete the survey. This survey is confidential!

Molina’s Provider Self Services has a variety of tools to simplify your transactions whether you need to check eligibility or check claim status. For more information contact Provider Services. Molina Healthcare is now collecting National Provider IDs (NPIs) from contracted and non-contracted providers. Submit your NPI to Molina via our Provider Self-Services Portal.

Please notify Molina Healthcare at least 30 days in advance when you have any of the following:

  • • Addition or closure of office location
