Thank you for your interest in joining Molina Healthcare of Wisconsin and My Choice Wisconsin’s (by Molina Healthcare) provider network. We provide coverage to members across a range of government-sponsored healthcare programs throughout the State of Wisconsin, including Medicaid, Medicare, and Long-Term Services and Supports (LTSS).
Our current programs include:
- BadgerCare Plus (Medicaid)
- Medicaid SSI (Medicaid)
- Family Care (Long-Term Care)
- Family Care Partnership (Long-Term Care with integrated Medicare-Medicaid)
- Medicare Advantage (Dual Advantage / D-SNP)
Contracted providers are an essential part of delivering quality care to our members, and we value our partnerships with our providers.
Important: To be eligible to contract with Molina Healthcare of Wisconsin, providers must be enrolled with Wisconsin Medicaid through the ForwardHealth Portal, when applicable.
Healthcare providers must have an active National Provider Identifier (NPI) registered with Wisconsin Medicaid that corresponds to the services being requested. Both individual Type 1 and organizational Type 2 NPIs must be properly registered and aligned to the provider’s enrollment record, as applicable.
LTSS providers, including certain atypical providers that do not use an NPI, must have the appropriate Wisconsin Medicaid enrollment, provider identification, or other required registration for the services being requested.

Are you a contracted Provider with Molina Healthcare and need to add practitioners to your existing practice?
Login to the Provider Network Management portal to add a practitioner or upload a roster.

Updating Provider Information
Are you a contracted Provider with Molina Healthcare and need to make changes to your provider information?
Please notify Molina Healthcare at least 30 days in advance when you have any of the following:
- Change in office location, office hours, phone, fax, or email
- Addition or closure of office location
- Termination of a provider
- Change in Tax ID and/or NPI
- Open or close your practice to new patients
Providers should submit their change requests to MHWIProviderNetworkManagement@Molinahealthcare.com
