Provider Portal Update
Beginning November 16, 2020, providers who are registered Availity subscribers can access all Molina of Wisconsin services through the Availity portal. Be advised that starting November 16, the Molina Call Center will not be accepting calls for claims status. The Availity portal should be used for all Molina of Wisconsin services.

Please notify Molina Healthcare at least 30 days in advance when you have any of the following:
- Change in office location,
office hours, phone, fax, or email
- Addition or closure of office location
- Addition or termination of a
- Change in Tax ID and/or NPI
- Open or close your practice to new
patients (PCPs only)
