At this time, Molina Healthcare of Wisconsin is temporarily not accepting new contract requests for providers due to ongoing integration activities with My Choice Wisconsin. This pause allows us to focus on enhancing and streamlining our provider network as part of our integration and restructuring efforts.

Our goal is to ensure a seamless, high-quality experience for our existing network while we complete necessary updates to our processes, systems, and contracts. This ensures that we continue to meet regulatory requirements, improve operational efficiency, and maintain the highest standards of care for our members.

We will reassess the status of our network after January 1, 2025. Please continue to check this page for updates on when we will begin accepting new provider applications.

Thank you for your patience and understanding as we work to strengthen our provider network. We look forward to reopening the application process and expanding our network of quality providers in the near future.

Important Reminder:

Please notify Molina Healthcare at least 30 days in advance when you have any of the following:

  • Change in office location,
       office hours, phone, fax, or email
  • Addition or closure of office location
  • Addition or termination of a
       provider
  • Change in Tax ID and/or NPI
  • Open or close your practice to new
       patients (PCPs only)
