Molina in Washington’s Disease Management Program
Molina’s Disease Management program offers support to Molina members diagnosed with prediabetes, diabetes, asthma, Chronic Obstructive Pulmonary Disease (COPD), Cardiovascular Disease (CVD), Congestive Heart Failure (CHF), depression and obesity.
Our team of Clinical Health Managers can help you manage your health in a way that is right for YOU. For example we can help you:
- Better understand your disease
- Monitor and manage your disease
- Answer questions about your disease or direct you to someone who can
- Talk with your provider to answer any questions you may have
- Improve and maintain healthy lifestyle changes
- Learn about your disease
- Remove barriers to care and refer you to needed services
Also, by working with your provider, family members and/or caregivers, we can help support you in managing your disease and staying healthy.
Who is Accordant?
Accordant Health Services (AHS) is a company that works with Molina to provide disease management support to members with rare complex chronic diseases including:
- Amyotropic lateral sclerosis (ALS)
- Chronic inflammatory demyelinating polyradiculoneuropathy (CIDP)
- Crohn’s disease
- Cystic fibrosis (CF)
- Dermatomyositis
- Gaucher disease
- Systemic lupus erythematosus (SLE)
- Multiple sclerosis (MS)
- Myasthenia gravis
- Parkinson’s’ disease
- Polymyositis
- Rheumatoid arthritis (RA)
- Scleroderma
- Seizure disorders
- Sickle cell disease
- Ulcerative colitis
If you have one of these rare chronic conditions, an Accordant RN may contact you and provide you with information about the program. If you are interested in participating, the Accordant RN will work with you and your assigned Molina Case Manager, if you have one, to make sure your health education and referrals are coordinated.

How is Molina’s Disease Management program different from Accordant’s program?
The programs are very similar. The main difference is that Molina’s clinical staff works primarily with members who have the following conditions: prediabetes, diabetes, asthma, COPD, CVD, CHF, depression and obesity while Accordant’s clinical staff works with those members with specific rare chronic conditions.
