Thank you for your interest in joining Molina Healthcare of Florida's provider network. Currently, we provide coverage to Medicaid and Marketplace members in the State of Florida. We are also honored to have been selected to serve the forthcoming Children's Medical Services Program. Contracted providers are an essential part of delivering quality care to our members, and we value our partnerships with our providers.

If you received a recruitment letter for Children's Medical Services Program, please follow the instructions below based on your provider type. When you submit your contract request through our online provider portal, please provide the reference number contained on the letter you received.

Note: To be a contracted, participating provider with Molina Healthcare of Florida for Statewide Medicaid Managed Care (SMMC) and the Children's Medical Services (CMS) Program services, you must be an enrolled Medicaid provider and have a valid and active Medicaid ID issued by the Agency for Health Care Administration.

    Long-term Care Program

     

    Florida Medicaid's Long-term Care Program provides home and community-based and institutional services to help individuals reside in their home and community.

    Vision Providers

     

    Molina covers both medical and routine vision services. Medical benefits are administered by Molina, while Marketplace and supplemental Medicare benefits are administered through our vendor, Vision Service Plan (VSP). To provide both medical and routine vision services to Molina members, you will need to contract with both Molina and VSP. To contract with VSP, please visit the provider page at VSP.com or by calling (916) 851-4165.

    Medicaid Providers can contract with iCare Health Solutions by calling (855) 373-7627 or visit myicarehealth.com for more information.

    Note: Ophthalmology physicians may contract directly through Molina. Please submit a Contract Request on our online provider portal.

     

    Note: Practitioner groups that include optometrists must be contracted with our vendor in order to contract with the Health Plan.

     

    Pharmacy Providers

Are you a contracted Provider with Molina Healthcare and need to make changes to your provider information?  Please notify Molina Healthcare at least 30 days in advance when you have any of the following:

  • Change in office location, office hours, phone, fax, or email
  • Addition or closure of office location
  • Termination of a provider
  • Change in Tax ID and/or NPI
  • Open or close your practice to new patients
