Molina Marketplace members in California can earn gift cards for completing certain health activities:

• $100 – Health Risk Assessment
• $50 – Annual Wellness Visit
• $50 – Colorectal Cancer Screening*
• $50 – HbA1c test*
• $50 – Blood Pressure Check (CBP)*

* To qualify, you must meet HEDIS™ measures. If you qualify, you’ll need to enter the date of your visit and the provider you saw in the rewards portal, found in your My Molina account under the Molina Healthy Rewards tab.

What is Molina Healthy Rewards?

Molina Healthy Rewards is a platform with programming to improve your health. The platform hosts free tools on topics like healthy eating, managing your cholesterol, how to quit smoking and more. You can get a gift cards by answering some questions about your health and visiting your doctor for your yearly check-up.

How to get your gift cards:

1. Log in to your My Molina account. Go to the My Wellness page and click the Molina Healthy Rewards tab.

2. Complete the Health Risk Assessment to earn a $100 gift card.

3. Schedule and attend your annual wellness visit to earn a $50 gift card. Need help finding a doctor? Use our search tool or call (833) 982-1452 (TTY: 711), Monday–Friday, 8 a.m.–6 p.m. local time.

4. After completing your visit and assessment, we’ll email you instructions to claim your gift card.

Please allow 6–8 weeks after we receive your claim for processing.
