Breathe with easeSM Asthma Program

Molina Healthcare provides an asthma disease management program called breathe with easesm, designed to assist Members in understanding their disease. Molina Healthcare has a special interest in asthma, as it is the number one chronic disease diagnosis for our Members. This program was developed with the help of several community providers with large asthma populations. The program educates the Member and family about asthma symptom identification and control. Our goal is to partner with you to strengthen asthma care in the community.


Healthy Living with DiabetesSM

Molina Healthcare has a diabetes health management program called Healthy Living with Diabetessm designed to assist Members in understanding diabetes and self-care. The Healthy Living with Diabetessm program is designed for members 18 years of age or older upon enrollment in the program. The member must have a confirmed diagnosis of diabetes, (non-gestational and/or non-steroid-induced). The member shall participate in the program for the duration of his or her eligibility with the plan’s coverage or until member opts out. Each identified member will receive specific educational materials and other resources in accordance with their assigned stratification level. Additionally, all identified members will receive periodic educational newsletters.


Heart Healthy LivingSM

Molina Healthcare has a heart disease health program called Heart Healthy Livingsm designed to teach members how to manage their heart disease. Each identified member will receive educational materials about heart disease, hypertension and/or congestive heart failure and ways to stay healthy. Additionally, all identified members will receive periodic educational newsletters



Molina in Washington’s Disease Management Program

Molina’s Disease Management program currently offers support to Molina members diagnosed with prediabetes, diabetes, asthma and COPD.

Molina’s RN Health Managers help members manage their health in ways that are right for them. For example, this customized program can help members:

  • Better understand their disease
  • Monitor and manage their disease
  • Answer questions about their disease or direct them to someone who can Reinforce and supplement the information given to them by their health provider
  • Partner with providers in developing and implementing a disease management treatment care plan
  • communicate, collaborate and coordinate with their provider(s) to ensure continuity of care and support them in managing their disease.
  • Update providers on progress and/or areas of concern, problems identified, and/or progress made
  • Remove barriers to care and refer members to needed services

We understand that resources for supporting patients in these ways may be limited. That is why we offer this voluntary program FREE OF CHARGE and welcome the opportunity to support you in caring for your Molina patient.

To refer your Molina patient to this program please complete and submit the attached referral form. To talk with a Health Manager about your patient, please call Molina’s Member Services department at (800) 869-7165 and ask for the Disease Management department or send an e-mail to MHWDiseaseManagement@MolinaHealthCare.Com. Molina patients can also call us directly to participate.

Who is Accordant?

Accordant is a health services company that is contracted with Molina to provide disease management support to members with rare complex chronic diseases including:

  • Amyotropic lateral sclerosis (ALS)
  • Chronic inflammatory demyelinating polyradiculoneuropathy (CIDP)
  • Crohn’s disease
  • Cystic fibrosis (CF)
  • Dermatomyositis
  • Gaucher disease
  • Hemophilia
  • Systemic lupus erythematosus (SLE)
  • Multiple sclerosis (MS)
  • Myasthenia gravis
  • Parkinson’s disease
  • Polymyositis
  • Rheumatoid arthritis (RA)
  • Scleroderma
  • Seizure disorders
  • Sickle cell disease
  • Ulcerative colitis

If a member is diagnosed with one of these rare chronic conditions, an Accordant RN will contact the member directly and provide them with information about the program. If interested in participating, the Accordant RN will work with the member, the member’s providers, and the member’s Molina Case Manager to ensure health education and referrals are coordinated.

This program is completely optional and free of charge to all Molina members.
