Home Health Services: Requests for Home Health care services require prior approval. Recipients must meet one or more criteria cited in (8.325.9 NMAC) Home Health services in order to receive this benefit. Requests for Home Health care are submitted on the MAD301 form. The approval is indicated on the MAD301 form by the presence of the authorization number, dates of service and number of types of visits. The updated form is then returned for notification to the requesting provider.
