EPSDT
EPSDT is a Medicaid benefit that provides comprehensive and preventive health care services for children under the age of 21 who are unable to perform some/all activities of daily living(ADL’s) or independent activities of daily living (IADL’s) because of disabilities or functional limitations. The goal of EPDST is to ensure that children and adolescents receive preventive screening for physical, dental, mental health, hearing, vision, developmental, and specialty services as well as diagnostic services for identified risks which are treated as appropriate. EPDST requests are received on the MAD 056 form. EPSDT recipients must meet criteria for EPDST Personal Care services, and approvals are indicated on the MAD 056 form by the presence of an authorization number. The updated form is then returned to the requesting provider
