¿Qué es una red de atención social?
Una Red de atención social (Social Care Network, SCN) ayuda a los miembros como usted con vivienda, alimentos, transporte a citas de salud, administración de la atención y más. Una SCN lo hace a través de un grupo de organizaciones comunitarias que trabajan juntas. Las SCN están aquí para "involucrarse totalmente" cuando usted está en uno de nuestros planes:

¿Cómo se tomará una decisión sobre si puedo usar una SCN?
Hay dos maneras de hacerlo. La primera es utilizar la herramienta de selección en el sitio web de una SCN. La herramienta le informará de inmediato si es elegible. La segunda forma es comunicarse con la SCN: Un “navegador” de LA SCN le hará algunas preguntas y revisará su elegibilidad, los servicios disponibles y le ayudará con los siguientes pasos.
