How to Enroll

Enrolling into South Carolina Healthy Connections Prime (Medicare-Medicaid Plan) is easy.

You must contact South Carolina Healthy Connections Choices to let them know you would like to enroll in our health plan, which you can do in two ways:

  • Call South Carolina Healthy Connections Choices Customer Service Center at (877) 552-4642 TTY: (877) 552-4670. Hours are from 8 a.m. to 6 p.m. Monday through Friday, excluding South Carolina state holidays. A member service representative will assist you. 

 

You have the right to pick a health plan that is best for you.

For more information, you can contact Member Services.

You may have to pay for services that are not covered. You may also have to pay for services from providers not part of our network. If the services were an emergency, you don’t have to pay. If you need help, call Member Services.
