Before you contact Molina, you must have your billing and rendering NPI’s registered with the Health Care Authority (HCA).http://www.hca.wa.gov/billers-providers/apple-health-medicaid-providers/enroll-provider Please note, all rendering and billing NPI numbers must be registered with the HCA. If you have questions regarding this state requirement, please contact the HCA at 1-800-562-3022 ext. 16137.

Washington law requires all health care providers submit credentialing applications through ProviderSource. To avoid delays please ensure applications are current, including work history and attestations. You must also grant Molina access to retrieve this application.

We will evaluate your submission and provide you with a response within 120 days of the next steps. Incomplete submissions will not be evaluated.

Please notify Molina Healthcare at least 30 days in advance when you have any of the following:
- Change in office location, office hours, phone, fax, or email
- Addition or closure of office location
- Addition or termination of a provider
- Change in Tax ID and/or NPI
- Open or close your practice to new patients (PCPs only)
Reminder: Molina is GOING GREEN as of July 1st and all claims need to be submitted through an EDI Clearinghouse or via Molina's Provider Portal. Please contact our Provider Services team if you have any questions.
