    One care manager serves as the point of contact and leads the interdisciplinary team.

    This whole-person approach eliminates barriers for both members and providers. Our care managers provide support and help members achieve their goals of improved health and self-sufficiency. Using a fully integrated and population-appropriate care management model, we engage members and providers to address each individual’s physical health, behavioral health, and social services needs.

  • 3. Our Integration Within the Community

    Molina was an early adopter of community health workers in the United States.

    Our community health workers assist members with finding housing, food, clothing, and transportation, and scheduling appointments. We match members’ needs with the strengths of our community partners. To reduce health disparities, we collaborate with community-based organizations to address unmet health needs, improve access to care, and promote health equity.

    Our goal is to be the most effective company delivering government-sponsored
    healthcare.
    Members: We employ innovative ways to engage members while focusing on
    addressing their questions, requests, and concerns.
