What is a Social Care Network?
A Social Care Network (SCN) helps members like you with housing, food, transportation to health appointments, care management and more. A SCN does this through a group of community organizations that work together. SCNs are here to “wrap around” you when you’re in one of our plans:

- Senior Whole Health
How will a decision be made on whether I can use an SCN?
There are two ways. The first way is to use the screening tool on an SCN’s website. The tool will let you know right away if you’re eligible. The second way is to contact the SCN: An SCN “Navigator” will ask you some questions and review your eligibility, available services and help you with the next steps.
When can I use my SCN?
Once you’re eligible, you can use an SCN right away. Below is a chart that shows you where you can find Molina Healthcare SCNs. Contact SCNs directly.
