Personal Attendant Service
Personal attendants are caregivers that help with activities of daily living and household chores necessary to maintain clean and safe home environments in community settings (except in adult foster care homes or assisted living facilities). Services may include protective supervision and help performing health-related tasks delegated by a registered nurse. The attendant helps with housekeeping items such as shopping, light housekeeping and laundry. The attendant can also help with hands-on personal care items such as bathing, dressing and feeding.

Nursing
This service focuses on long-term needs rather than short-term acute healing needs, such as weekly insulin syringes or med-set set up for members unable to do this for themselves. The goal is to restore and maintain a member’s maximal level of function and health. These services are provided instead of hospitalization or a nursing facility stay. A doctor’s order is required for this service.

Emergency Response Services
This electronic equipment allows members 24-hour access to help in an emergency. The equipment is connected to your phone line to call the response center and/or other forms of assistance once the help button is pressed.

Home Delivered Meals
Prepared food brought to the member’s home that may consist of a heated lunch, dinner or both, which can be refrigerated and eaten later. This service is designed for the member who cannot prepare his or her own meals but is able to feed him or herself.

Minor Home Modifications
These are physical modifications to a member’s home. The modifications must be necessary to support the health, welfare and safety of the member while enabling them to function with greater independence in their home. Without the modification, a member would require some type of institutionalized living arrangement, such as a nursing facility or assisted living. Adaptations that do not help the member’s safety or independence are not included as part of this service, such as new carpeting, roof repair, central air or home additions.

Adaptive Aids, Medical Equipment and Medical Supplies
This service includes devices, controls, appliances or medical equipment and supplies that help people live more independently. These will enable the member to increase his or her abilities to perform activities of daily living, or to perceive, control or communicate with the environment in which they live.

Dental
Services provided by a dentist to preserve teeth and meet related medical needs. Services do not include cosmetic dentistry.

Physical, Occupational and Speech Therapy
Services designed to improve and or restore a person's functioning: includes physical, occupational and/or speech therapy. A doctor’s order is required for this service.

Adult Foster Care
A 24-hour living arrangement in an adult foster care home enrolled with DADS. Services may include meal preparation, housekeeping, personal care, and help with transportation and activities of daily living. Individuals pay room and board.

Assisted Living
A 24-hour living arrangement in a licensed personal care home. Services may include personal care, home management, social and recreational activities, 24-hour supervision, help with the administration of medicine, and transportation. Individuals pay room and board.

Transition Assistance Services
Services to help people transitioning from a nursing home or institutional setting into a home in the community by paying for one-time costs of setting up their new home.

Financial Management Services
Services that help people using the Consumer Directed Services option manage their payroll and budget.

Respite
This service provides relief for unpaid family or primary caregivers who are meeting all the needs of the member. The respite caregiver assists the member with all daily needs when the family or primary caregiver is absent. Respite can be provided by a homemaker, personal assistant service, nurse or an adult day health center.

Protective Supervision
Protective supervision is watching over the member while the primary caregiver is out of the home. It does not require the delivery of personal care tasks. Protective supervision is appropriate when it is necessary to protect the member from injury due to his/her memory loss and/or physical weakness.

Cognitive Rehabilitation Therapy
This therapy can help a person learn or relearn cognitive skills that have been lost. This may be the result of damage to brain cells. It can be offered in an outpatient setting or in your home. You can speak with your doctor to find out if you can be tested and are you able to get this service.

Employment Assistance
Help locating paid employment in the community by helping identify interests, evaluating skills and determining needed supports. Also help locate potential employers.

Supported Employment
Provides ongoing support at a worksite to help an individual keep their job. Services include adaptations, supervision and training, based on the person’s support needs.
