You must meet your state’s rules, including income limits.

You must live in the state where you apply.

Bring papers that show:

• Who you are (ID, birth certificate, Social Security card)
• Where you live (lease, utility bill, driver’s license)
• How much money you make (pay stubs, tax forms, benefit letters)
• Citizenship or immigration status (passport, green card)
• Household details (name and birth date for each person in your home)

Every state is different. Check with your state Medicaid office to see the exact list.

Some groups may have extra rules, like children, people who are pregnant and parents.Some states have programs for people with serious health needs, even if they do not meet every rule.
