Member programs and services

Molina offers many programs to help you stay healthy. These services may be available in your state.

Health management programs

If you have a long-term health condition, you can get no-cost information, tips and support.

Programs include:  

  • Breathe with Ease® Asthma Program  
  • Healthy Living with Diabetes® Program  
  • Living with Chronic Obstructive Pulmonary Disease (COPD) Program  
  • Heart Healthy Living Program  

How do you join?

  • You may be added automatically (in some states; varies by plan).
  • A nurse or care manager may call you.
  • Your doctor can refer you.
  • You can join or leave anytime.

Breathe with Ease® Asthma Program

This program teaches you how to control asthma.

You will learn:

  • What causes asthma
  • Early warning signs
  • How to take your medicine
  • How to work with your doctor
  • How to use an inhaler with a spacer

You may get:

  • Asthma learning materials
  • Help with rides and scheduling (in many states; benefits vary by state and plan)
  • Asthma care tips
  • Referrals to resources
  • A newsletter

Joining

You may join automatically if you have asthma, or your doctor can refer you.
You can join or leave anytime.

Healthy Living with Diabetes® Program

This program helps people with any type of diabetes.

You will learn:

  • About different types of diabetes
  • How to care for yourself with food, exercise and medicine
  • How your PCP and other team members can help

You may get:

  • Diabetes learning tools
  • Help with rides and scheduling (in many states; benefits vary by state and plan)
  • Care tips
  • Referrals to community programs
  • A newsletter

Joining:
If you qualify, you may be added automatically (in some states; varies by plan).
Your doctor can also refer you.

Living with COPD Program

This program helps adults with COPD stay healthy.

You will learn:

  • How to stop smoking
  • How to work with your doctor
  • Early signs of COPD flare‑ups
  • How to take your medicine

  • COPD care tips

  • Heart‑health care tips
  • Referrals to community services

These programs help you make healthy choices.

Programs include:

Weight‑loss program

Learn about healthy food and exercise.

Stop smoking program

Get no-cost learning tools and one‑on‑one help from a counselor.
You may get nicotine replacement if it is right for you.

Pregnancy program

Get help with:

  • Phone counseling
  • Workbooks and learning tools
  • Social service support
  • Nurse care management
  • Tips to stay healthy before and after birth

You can also ask for other health education materials.
