Molina Care Connections helps you stay healthy. Our friendly health care team talks with you about how you feel and what you need. A licensed provider can visit your home or meet with you by video. You can receive care without leaving your home.
We help with yearly checkups, staying healthy, and recovering after a hospital stay. We can also help when you need extra support or cannot see your regular doctor. We want to make sure you get the care you need when you need it.

Care Connections is part of the Molina Healthcare family. We help adults and children with many health needs. We provide care to members in Medicare, Medicaid and Marketplace.
If you are in a Molina health plan, Care Connections visits are part of your benefits. There is no extra cost for these visits. Some additional services may have a small cost, depending on your plan.
