If you are a dual beneficiary who qualifies for both Medicare and Medi-Cal and you do not want to join Cal MediConnect, a dual Medicare and Medicaid plan, at any time you may choose to disenroll or "opt-out". You can opt-out from the Cal MediConnect dual Medicare and Medi-Cal program and go back to Original Medicare or Medicare Advantage plan. If you do not join a Cal MediConnect dual program plan, you must join a Medi-Cal plan for Long-Term Services and Supports, such as Molina Healthcare Medi-Cal
You can change health plans any month. To do this, call Health Care Options at (800) 430-4263; TTY: (800) 430-7077, or call Molina Healthcare Member Services and let us know that you want to leave this plan.
For your Medicare coverage, you can choose a Dual Special Needs Plan (D-SNP) such as Molina Medicare Complete Care (HMO SNP)
. Medicare Fee for Service (FFS), or a Medicare plan (Medicare Advantage) with another health care provider.
The Health Insurance Counseling and Advocacy Program (HICAP) provides enrollment and options counseling to Medicare beneficiaries.
