How to Enroll

To enroll in Medicaid on your own, call Utah State Medicaid at (801) 526-9422 or (866) 608-9422 or you can call Molina and we would be happy to assist you.

  • Call us at (866) 504-7242 and we will help you through the steps. We can also help you change your plan.
  • You can also fill out the "Have a Question" form at the lower left corner of this webpage and we will call you.
