Begin the process of joining our network of quality providers by completing a Contract Request Form. We will evaluate your submission and provide you with a response within 5-7 business days for next steps.

Please Note: To be a contracted provider with Molina Healthcare of Michigan, you must have your NPI registered with CHAMPS. (Link to CHAMPS).

Molina’s Provider Self-Services Portal has a variety of tools to simplify your transactions whether you need to check eligibility or check claim status. For more information contact Provider Services. Molina Healthcare is now collecting National Provider IDs (NPIs) from contracted and non-contracted providers. Submit your NPI to Molina via our Provider Self-Services Portal.

Reminder: Molina is GOING GREEN as of July 1st and all claims need to be submitted through an EDI Clearinghouse or via Molina's Provider Portal. Please contact our Provider Services team if you have any questions.​​
