Appeals and grievances

As a Molina Healthcare member, you have the right to share concerns about your care or our services. This is called a grievance.

For example, you can file a grievance if you have concerns about:

  • The care you get from your provider
  • How long it takes to get an appointment or see a provider
  • Which providers you can choose

You can file an appeal when you do not agree with our decision to:

  • Stop, change, suspend, reduce or deny a service
  • Deny payment for services you received

You have several ways to file an appeal or grievance:

  • Call Molina Healthcare at (855) 885-3176 (TTY:711), Monday through Friday, 8 a.m. to 6 p.m. local time. We will try to help you over the phone.
  • Fill out the Member Grievance Form and mail it to: Molina Healthcare of South Carolina, Inc., Attn: Grievance and Appeals Unit, PO Box 182273, Chattanooga, TN 37422. Or fax it to (866) 713-1891.
  • Log in to the My Molina member portal

We will send you a letter acknowledging receipt of your appeal or grievance within five calendar days and issue a formal response within 30 calendar days.

You can also call the South Carolina Department of Insurance (SCDOI) at (803) 737-6180 or (800) 768-3467 for help at any time.
