Grievances

If you have a problem or concern with your medical care or our services, you have the right to file a complaint. At Molina, we refer to a complaint as a grievance. Some examples of grievances are:

  • The care you get from your provider.
  • The time it takes to get an appointment or be seen by a provider.
  • The providers you can choose for care.

Appeals

You can file an appeal when you do not agree with our decision to:

  • Change, suspend, or reduce a service.
  • Stop or deny a service.
  • Deny payment for services provided.

How to file a grievance or appeal:

You can file a grievance or appeal by:

  • Phone. Call Member Services at (844) 809-8445 (TTY/TDD: 711) Monday – Friday 8 a.m. – 6 p.m. local time if you need any help with the forms.
