Your plan materials

You can request printed or electronic copies of member materials, including provider directories, member handbooks, provider network adequacy standards, and appeal and grievance notices. Materials can be provided in other languages by request. You may ask for it in braille, large print, or audio at no cost to you. If you are hearing or visually impaired, special help can be provided at no cost to you. Please call Member Services at (888) 898-7969 to request a copy of member materials free of charge.
