Grievances

If you have a problem or concern with your medical care or our services, you have the right to file a complaint. At Molina, we refer to a complaint as a grievance. Some examples of grievances are:

  • The care you get from your provider
  • The time it takes to get an appointment or be seen by a provider
  • The providers you can choose for care

Appeals

You can file an appeal when you do not agree with our decision to:

  • Change, suspend, or reduce a service
  • Stop or deny a service
  • Deny payment for services provided

How to file a grievance or appeal:

You can file a grievance or appeal in the following ways:

  • Phone: Call Member Services at (844) 862-4543 (TTY: 711), Monday–Friday, 8:00 a.m.–5:00 p.m. local time.
