Your Medicaid Eligibility and Enrollment in Ohio

Ohio Medicaid helps people get no-cost health care.

You may get Medicaid if you:

• Live in Ohio
• Meet income requirements
• Are a U.S. citizen, a lawful resident, or have legal immigration status
• Are in at least one of these special groups:
• • 65 or older
  • Pregnant
  • Infants and children
  • Blind or have a disability

What you may need to sign up or renew:

• Job and wage info for you and your family (pay stubs, W‑2s or tax forms)
• Info about any job‑based health insurance available to you and your family
• Policy numbers for your current health insurance
• Social Security numbers (or document numbers for legal immigrants)

Ways to sign up or renew:

1. Online: Go to benefits.ohio.gov. Log in or create an account and send in your form.
2. By phone: Call the Ohio Medicaid Consumer Hotline at (800) 324-8680, option 8 (TTY 800-292-3572), Monday to Friday, 7 a.m. to 8 p.m. and Saturday 8 a.m. to 5 p.m. Eastern Time (ET).
3. By mail: Fill out the form and mail it to your local County Department of Job and Family Services (CDJFS) office.
4. In person: You can drop off forms or fill them out at your local CDJFS office.

When you sign up for Medicaid, you can choose Molina Healthcare to get your health care benefits.

Medicaid renewal

Most people need to renew their Medicaid eligibility with CDJFS every 12 months. If you need to renew, you will receive a packet in the mail. It is important that you keep your address information up to date with CDJFS. If you do not, you could lose your benefits.
