How Medicaid eligibility works

The Utah DWS decides who qualifies. To see if you do, sign up online in myCase at jobs.utah.gov/mycase or call DWS at 1-866-435-7414.

If you live in Box Elder, Cache, Rich, Weber, Tooele, Davis, Morgan, Salt Lake, Summit, Wasatch, Utah, Iron, or Washington counties, you must choose a health plan. You can choose Molina Healthcare.

How to enroll:

  1. Check eligibility with DWS online or by phone.
  2. Sign up and pick Molina as your health plan. You can also get help from a Medicaid Health Program representative at 1-866-608-9422.
  3. Need help from Molina? Call us at 1-866-504-7242.
  4. After you enroll, watch for your welcome packet and ID card in the mail.
